Birth: Complete history of pregnancy, birth and
development, physical exam, measurement of weight. length
and head circumference. Notation of any developmental
or hereditary diseases, PKU and T4. Hepatitis series may
be started at birth or any age.
Age 4 weeks: History and physical, height, weight,
developmental appraisal, discussion and counseling regarding
feeding, car seats and health maintenance, measure head
circumference and some estimation of vision.
Age 2-3 months: History and physical, height,
weight, developmental appraisal, discussion and counseling,
head circumference and vision. Immunizations: first DPT,
oral polio and HIB.
Age 4-5 months: History and physical, height,
weight, developmental appraisal, discussion and counseling,
physician dental screening. Immunizations: second DPT,
oral polio and HIB.
Age 6-7 months: History and physical, height,
weight, developmental appraisal, physician's dental screening,
head circumference, vision, hearing. Immunizations: third
DPT and oral polio.
Age 12 months: History and physical, height, weight,
head circumference, developmental appraisal, discussion
and counseling, especially in regard to poisoning and
accident prevention, physician's dental screening, tuberculin
skin test and Hct., sickle cell screening if needed.
Age 15 months: History and physical, height, weight,
developmental appraisal, discussion and counseling, physician's
dental screening, head circumference. Immunizations: MMR
at 12-15 months, HIB, DPT.
