New
Patient Information Sheet
For appointments, please call the clinic at 573-686-2411 and a member of our staff will
get back to you to confirm a time and date.
Arriving at the Clinic
The Kneibert Clinic building has been carefully planned
with you, the patient, in mind. When entering the building,
register with the receptionist. She will direct you to the
appropriate area of the clinic in which you are registered.
Accessibility
Wheelchair access is available on our second level
parking lot. An elevator is available in the building for
access to other floors.
Medical Records
Beginning with your first visit, a single medical
record will be prepared for each patient. This record is available
to your doctor when you are receiving care. Its contents are
always treated with the strictest confidence. New patients
may want to print out our New
Patient Information Sheet, fill it out at home and bring
it in.
All medical records, including x-rays and reports, are the
permanent property of Kneibert Clinic. Extracts or copies
will be made available to other physicians upon written request
of the patient.
Financial Arrangements
Kneibert Clinic asks that services be paid for when
they are rendered. If payment in full is not possible, payment
arrangements may be made at the Business Office before leaving
the clinic.
Itemized statements are sent out monthly. Accounts are due
and payable upon receipt and delinquent in 30 days unless
other arrangements have been made.
Insurance
When registering, you will be asked to provide insurance
information. Kneibert Clinic files your insurance claims for
you.
Medicare
The clinic is a participating provider in the Medicare
program. Claims are filed automatically for you. You are responsible
for the deductible, 20 percent co-payment or other non recovered
services.
Managed Care
The physicians at Kneibert Clinic participate in
many managed care plans. Please refer to your provider directory
to determine eligibility.
